Medication errors are one of the most common — and most preventable — causes of hospitalization for older adults. If your loved one is taking five or more prescriptions, a simple "reminder" is rarely enough. They need true medication management.
Reminders vs. administration vs. management
At AllGrace Living we provide all three — reminders, administration, and management — for every resident who needs it.
- Reminders: a staff member says, "Time for your pills." The resident manages the bottle, the dose, and the timing.
- Administration: a trained staff member prepares the dose, hands it to the resident, watches them take it, and documents it.
- Management: the home tracks every prescription, refills proactively, coordinates with the pharmacy and physician, and flags interactions or side effects.
What good medication management looks like day to day
- A licensed Med Tech or Med Aide on every shift
- Each dose double-checked against the physician's orders
- Every dose timestamped and initialed in a Medication Administration Record (MAR)
- Direct communication with the pharmacy for refills and changes
- Family notified of any new prescription, side effect, or hospitalization
Why this matters in a small adult care home
Because we serve a small group of residents, our Med Tech actually knows your mother — her baseline, her quirks, the way she winces when her hip flares. That context catches problems weeks before a chart review ever would.
